چکیده

A number of different approaches have been utilized in recent years for the treatment of Parkinson's disease with varying effects on associated communication disorders. In particular, the drug treatments based on levodopa therapy and, more recently, deep brain stimulation of the sub-thalamic nucleus (STN-DBS) have yielded variable effects on the speech disorders exhibited by the majority of persons with Parkinson's disease. With respect to STN-DBS, although reports indicate significant improvement in the symptomatology in Parkinson's disease associated with limb function, the majority of reports suggest that the procedure has either minimal or, in most cases, negative effects on speech abilities of the affected individuals. By far the most effective speech therapy technique applied to dysarthria in Parkinson's disease reported to date is the Lee Silverman Voice Treatment (LSVT) program. In addition to speech impairment, in recent years there has also been increasing awareness that Parkinson's disease is also associated with the occurrence of language disorders, especially in its later stages. This special issue contains 13 papers of which 10 are related to issues associated with speech impairment and 3 further papers explore aspects of language impairment in Parkinson's disease. The first paper entitled " Perceived changes in communication as an effect of STN surgery in Parkinson's disease: a qualitative interview study " by E. Ahlberg et al. explored the perspective of four individuals with Parkinson's disease on the way their speech and communication changed as a result of STN-DBS. Overall the study confirmed earlier findings of negative effects on speech production; however, irrespective of this, the patients in general were positive as to the outcome of their surgery. The paper by F. Karlsson et al. " Deep brain stimulation of caudal zone incerta and subthalamic nucleus in patients with Parkinson's disease: effects on diadochokinetic rate, " investigated the outcome of STN-DBS and stimulation of the caudal zone incerta (cZi-DBS) on articulation rate and quality of plosive consonants in two articulatory diadochokinetic tasks. Whereas patients receiving STN-DBS increased in articulation rate with no effect on production quality, patients receiving cZi-DBS decreased in articulation rate and further showed reduction in production quality suggesting that cZi-DBS is more detrimental for extended articulatory movements than STN-DBS. The third paper, " Deep brain stimulation of caudal zone incerta and subthalamic nucleus in patients with Parkinson's disease: effects on voice intensity " by S. Lundgren et al. compares the effects of cZi-DBS and STN-DBS on voice …

Dopaminergic basis for impairments in functional connectivity across subdivisions of the striatum in Parkinson's disease.

The pathological hallmark of Parkinson's disease is the degeneration of dopaminergic nigrostriatal neurons, leading to depletion of striatal dopamine. Recent neuroanatomical work has identified pathways for communication across striatal subdivisions, suggesting that the striatum provides a platform for integration of information across parallel corticostriatal circuits. The aim of this study wa...

Behavioral and neurophysiological correlates of striatal dopamine depletion: a rodent model of Parkinson's disease.

UNLABELLED Both limb and cranial motor functions are adversely impacted by Parkinson's disease (PD). While current pharmacological and surgical interventions are effective in alleviating general limb motor symptoms of PD, they have failed to provide significant benefit for cranial motor functions. This suggests that the neuropathologies mediating limb and cranial motor impairments in PD may dif...

P 49: Exercise Effects on Cognitive Impairments Through Altering Neuroinflammation

Cognitive impairments describe a state of diminished or impaired mental and/or intellectual function such as Alzheimer's disease, Huntington's disease and Parkinson's disease. As these disorders are more frequent in the elderly and due to the ageing of population, serious attention must be paid to these diseases. Exercise has shown to have preventive and therapeutic effects on cognitive impairm...

Dynamic emotion processing in Parkinson's disease as a function of channel availability.

Parkinson's disease (PD) is linked to impairments for recognizing emotional expressions, although the extent and nature of these communication deficits are uncertain. Here, we compared how adults with and without PD recognize dynamic expressions of emotion in three channels, involving lexical-semantic, prosody, and/or facial cues (each channel was investigated individually and in combination). ...

Behavioural aspects of a modified crosstalk between basal ganglia and limbic system in Parkinson's disease.

Dysfunctions in dopaminergic neurotransmission lead to motor symptoms and cognitive impairments associated with behavioural disturbances. Parkinson's disease is a neurodegenerative disorder which is primarily characterized by an abnormal basal ganglia activity. Recently, increased attention has been directed towards the hippocampus in the development of non-motor symptoms. Given the temporal pr...

The association between fear of falling and quality of life for balance impairments based on hip and ankle strategies in the drug On- and Off-phase of patients with idiopathic Parkinson' disease

Background: Despite the negative effect of fear of falling during functioning and social participation of patients with Parkinson' disease, so far, only few studies have investigated its effect on the quality of life in these patients. We aimed to investigate the association between fear of falling and quality of life controlling for balance impairments based on hip and ankle strategy in drug O...
